Immunisation History Statement for Overseas Records

Families arriving from overseas must provide an Immunisation History Statement from the Australian Immunisation Register (AIR) when enrolling their child in early childhood education and care services or primary school.

If your child was immunised overseas, our Immunisation Team can assess and transcribe their overseas immunisation records, report vaccines to the AIR, and develop a catch‑up plan to ensure they are fully immunised for age under the National Immunisation Program.

There may be a cost associated with assessing overseas immunisation records. The Immunisation Team will review your child’s documents and advise you of the fee before proceeding with any assessment or reporting.

  • Your child’s overseas immunisation record
  • Parent name and phone contact

Catch up vaccines, if required can be given at our Community Immunisation Sessions, further information including any cost will be provided to you after Immunisation Statement has been completed.
