Service Mapping and Referral Pathways

A Resource to Services for Early Years Professionals

Mornington Peninsula Best Start in conjunction with Frankston City Council.

Thank you for using the Mornington Peninsula Best Start Service Mapping and Referral Pathways Resource (the Resource). The Resource is targeted at Early Years Professionals working on the Mornington Peninsula and in the Frankston municipality working with children aged 0 to 8 years and their families.

This resource has been made possible through the work undertaken by the Frankston and Mornington Peninsula Referral Pathways Working Party.  This dedicated group of professionals have been the driving force behind raising awareness of the importance of early intervention and promoting effective referral pathways.

Professionals identified the need for a resource to support them to navigate the complex early years’ service system and to make appropriate referrals for children  who may require a broad range of professional services and programs.

We hope you  find the Resource is useful and supports the work you undertake with children and families.
