Intake and Assessment

Residents eligible to receive Home and Community Care (HACC) services are older people and people with disabilities and their carers who are assessed as needing support to live at home, or to care for a family member at home.
Intake and Referrals
A referral must first be made to the Aged Services Asessment/Intake for Frankston City residents requiring an assessment for services.
A referral for services may be made directly by:
- Residents, their families or friends
- Medical and health practitioners
- Hospitals and other community agencies

- Further information on Home and Community Care services
- Making a referral for services

- Residential aged care facilities

- In home and residential respite and other services for carers of younger people with a disability
Assessment
After a referral has been received and processed, an assessment officer will make a time to visit each person to discuss and assess his or her needs. Priority is given to those at risk of residential care.
A comprehensive assessment is provided along with referral to external agencies that can strengthen an individual's capacity within their home and community.
Frankston City Council can arrange for interpreters so inquirers and clients can communicate in their language. Interpreters must be booked at least a week in advance of assessment interviews.
Changing or Cancelling Services
Information on how clients can change or cancel services and whom to contact will be given out at assessment. Clients are encouraged to contact Aged Services (03) 9784 1933 regarding any changes or cancellations to their services as soon as possible if they need to cancel or change one or more services for a short term, indefinitely or permanently.
Clients are welcome to resume services after a break, however, if there is a break in service of over three (3) months, a client will require re-assessment.
